Up until this point, the focus has been on a volume-based, or fee-for-service, care model. For example, a physician sees a patient in an Arbor Health clinic. The physician perhaps orders diagnostic tests, makes the diagnosis and then prescribes the appropriate medication or therapy. Once completed, our billing office sends a bill for services. The more patients we see, the more we bill. But what if that patient doesn’t take his antibiotic as prescribed? Or doesn’t understand the importance of following a new recommended diet? That patient is not going to experience the outcome his physi- cian was aiming for. In fact, that patient might return for care for the same problem. This is one reason behind the creation of value-based care. The value-based care model Value-based care is designed to expand the current doctor-to-patient relationship into a care team-to-patient relationship. Instead of your physician being the sole provider, your care team might also in- clude a social worker, a diabetes educator, a pharmacist or a psychologist. And under this model, the clinic will be paid a set amount per patient per year, regardless of the num- ber of clinic visits you have. Value-based care enables physicians and care teams to spend more time with their patients and help them truly understand how they contribute to their own wellness. The power of prevention Value-based care is also heavily focused on preventive services—keeping patients well by anticipating and preventing chronic disease. While health care is beginning to move toward that value-based model of care, physicians are still currently required to do fee-for-service care. It’s a tricky place to be. Thus, in the meantime, we have created a new care manager position in the clinics. It will be the care manager’s role to make sure patients are current on their screen- ings. For instance, is the patient due for a mammogram or colonoscopy? If they have diabetes, are they getting their A1C tests? Care managers make sure patients are get- ting the preventive care they need. We do not know when health care will fully adopt the value-based care, but we are getting ready. We are working to pre- pare for tomorrow—and, at the same time, doing our very best to care for our patients today. We recently welcomed a full-time speech therapist to our Rehabilitation Department, Christina Foxley, MA, CCC-SLP. Christina is nationally certified and state licensed to address most speech therapy needs. Common referrals for speech therapy include: ●   ● Swallowing disorders (e.g., dysphagia). ●   ● Developmental delays (e.g., speech sounds, amount/use of language, cognition). ●   ● Neurological disorders (e.g. traumatic brain injuries, stroke, Parkinson’s disease, etc.). ●   ● Fluency disorders (e.g., stuttering). ●   ● Autism spectrum disorder. ●   ● Voice disorders.
